All children attending Skate Night must have a current Medical Release Form on file.  The form is valid for one school year (September 1 – August 31).  We require one form per child.  The form must be completed by each child’s parent or guardian. 

To avoid delays on Skate Night; you may download the .pdf version of the release form below, print it, and have it notarized in advance.   There will be a notary available at each Skate Night event.  **If you plan to have your form notarized during the skate night event, please bring your valid driver’s license or other government issued ID with you as a form of identification.
